Here’s my offering, they’re only mine
I’m still in this body that doesn’t steal

Please answer these statements with a simple yes or no
I’ve tried to harm myself and/or tried to take my life
I would never think about ways of committing suicide
I feel optimistic and hopeful about my future
I believe that the reasons for living are greater than the reasons for dying
I would feel so bad that I would not mind if something happened to put my life
in danger
I would feel like dying
I would make specific plans to commit suicide
In this sort of situation, it is unlikely I would dare to commit suicide
I would begin to make preparations to commit suicide
I feel that I have reasons to live
I would not take any steps that were necessary to save my life
